Basic Insurance Information

Nascott participates with most insurance providers for payment of our services.
We will need to have a prescription from your Primary Care Physician or other appropriate healthcare professional.  Please check with your insurance provider to determine if a referral is needed.

Please be sure to bring the following items to your appointment:

  1. Insurance Card
  2. Photo ID
  3. Referral (as needed)

Nascott will work with you and your insurance company to obtain authorization for your service, but every insurance policy is different. This list of questions that you should ask of your insurance policy before your appointment will help you understand your insurance benefits.

  1. Does your policy have coverage for orthotics, prosthetics and/or pedorthics?
    • If yes, is there a benefit maximum (i.e. how much will the insurance pay each year/lifetime for orthotics, prosthetics and/or pedorthics)?
    • What time frame is considered by the insurance company when reviewing benefit maximum?
  2. How frequently are you allowed to receive the service being requested (i.e. one pair of shoes per calendar year, etc.)?
  3. Do you have a deductible?
    • If yes, has the deductible been met this year?
    • Is the deductible year a calendar year?
    • Is there a possibility that the service will not be complete until the beginning of the next deductible year?
  4. Will you have a copay?
  5. Is Nascott “In” or “Out” of network?
    • If “Out” of network, what is the financial impact to you?
    • Do I need a referral to be seen?

Some insurance companies take up to three weeks to provide authorization for service. Your device cannot be fabricated until the authorization has been received.
